Promo edition of the 4x3" CD by Merzbow on Ant-Zen from 2003. While the original came in a leather bound time-planner, this promo edition has the four CDs tucked on a cardboard square with a printed paper cover (see scan). Inside cover has a page of text. The CDs themselves are most likely never played. The package has some storage wear though. Indentation of the CDs in the paper cover, creating something like four small rings (not really visible very well on the scan), so I would give the packaging a VG or VG+, while the CDs are NM or M.
The CD looks flawless - Printed cover must have gotten damp on the top above the title (see scan), the generic plastic sleeve is a bit ripped up and dirty, but this could be replaced. I tested the CDr and it plays perfectly.

Compilation CD from 1990 for North America, issued by Wax Trax! from Chicago. With material from Techno Primitiv, Love and Lust, Trance, and Heartbeat, plus an unreleased track. CD in excellent condition. Printed parts of cover and inlay also excellent. Generic jewel case is a bit worn, but can be exchanged.
